In order for your subscription to be taken into account, it is mandatory that you send it back filled and signed with your payment (possibly several checks) payable to :

EEDF SARTROUVILLE

Without the form with your signature, your child will not be covered by the insurance policy of our Movement and will not be allowed to participate to our activities.

You must also fill, sign and send the parent's permission form and the sanitary form .
