Please fill it and send it back to us, in order to enroll as an Official / person in charge of Group's activities, as a Member of Group's activities (child, teenager, young adult) or as a Friend (parent, former scout...).

For a subscription to be taken into account, it is mandatory that you send it back to us filled and signed with your payment (possibly several checks) payable to:

Without the form with your signature, the member will not be protected by EEDF's insurance and could not participate to our activities.

For minor children, you must also fill, sign and send back the parental's permission form and the sanitary form.
